Clinical Observation of Blunt Abdominal Trauma

Abstract
This report is consist of a review of 121 patients who sustained blunt trauma on the abdomen during
the years 1963¡­1971 and who were treated on the surgical service of Capital Armed Forces General
hospital.
The causes of blunt abdominal trauma were car accident (54 cases), Blow (41 cases), and fall (21
cases), of which car accident was most common.
The clinical manifestation was abdominal pain, tenderness, rebound tenderness muscular rigidity,
abdominal distension, diminished bowel sound and etc.
Isolated injuries were not common, usually being associated with the other organs in the peritoneal
cavity.
In blunt abdominal trauma, the common organs injured were small bowel, spleen, liver, pancreas,
kidney and in decreasing other of frequency.
Abdominal tapping and radiography were helpful in the diagnosis of blunt abdominal trauma.
Simple closure was possible in minute injuries to the hollow viscus and liver, and splenectomy and
nephrectomy were particularly preferable in injury of spleen and kidney.
The common post operative complications were wound infection, pulmonary problems and fistulas,
of which wound infection was most common.
The main cause of ddeath were irreversible shock, septicemia, renal failure and pancreatitis, and death
of patients totalled 10 of 121 cases (8.2%).
